Eye prescription

Here are the most common abreviations found on a receipt of your optician.

d=distant (vision) n=near (vision)
od/os
od=right os=left 
S, Sph, Pwr
Spherical power, usually for far distances unless otherwise stated
C, Cyl 
Cylinder/ cylindrical power
axis, X
axis rotation of cylinder, between 0 and 180 degrees
Add, addition 
Reading addition, it is added to the far distance measures to get the reading powers
N(v)(od/os)
Near vision power 
Sometimes it's underneath the spherical data without any label.
This is the final reading power, that is with the addition already added.
reading power=far power + reading addition(add.)
PD

PD(R/L) of PD(od/os)
PD(d/n)
PD H(od/os)
PD=Pupil Distantie, the distance between the pupils in millimeters.
Pupil Distance for each eye seperately.
Pupil Distance for distant and near seperately
H=Pupil height from bottom lens (seperately)
SH 
Segment Height, the height where the transition should be between far and near powers
Pr(ism)./base
Data for prismatic correction.
V, vis. visus
Visus/vision, a measurement to indicate the quality of eye sight of the eyes. Higher is better.
